Hospital admission to discharge
Coordinate the transition to community supports
The Virtual Care Collective assists participants, families and authorised professionals with disability-related coordination before admission, during an agreed hospital stay and when preparing to return to the community. Suitable virtual services are available across Australia.
An agreed engagement may include organising information and questions; liaising with hospital teams, discharge planners and community providers; clarifying responsibilities, service gaps and practical barriers; coordinating agreed supports, equipment and accommodation readiness; and documenting next steps and handover arrangements.
Hospital clinicians retain responsibility for admission, treatment, medical decisions and discharge. We do not determine discharge readiness, prescribe, change treatment or replace local direct care. Coordination cannot guarantee a discharge date, placement or funding.
Enquire about availability. Timing, suitability, consent, authority, scope, funding and fees are confirmed before work begins. Urgent or after-hours coordination is offered only where specifically agreed. For immediate danger or urgent medical assistance, call 000. VCC is not an emergency response service.
As an unregistered NDIS provider, VCC works with eligible self-managed and plan-managed participants. Any intended NDIS funding is checked for the actual coordination activity and claiming conditions. Hospital treatment, clinical care and ordinary hospital responsibilities remain with the health system and are not charged to an NDIS plan. Only eligible disability-related coordination is considered for NDIS funding.
Private health consultancy is a separate option for individuals, families, providers and organisations, including people without an NDIS plan. A written proposal confirms scope, deliverables, fees and payment terms before work begins. See Fees & funding for enquiry and NDIS funding information.
Start with a brief enquiry and your email address and phone number. Referrers should provide their own contact details and leave participant identifiers, records and sensitive information out of the initial form. We agree an appropriate transfer method before records are shared.
