Medical record translation

For NDIS applications & reviews

Make the most of your medical evidence. VCC translates complex medical language into clear, structured summaries for NDIS applications, plan reassessments and reviews. We bring documented impacts on daily life and support needs into focus, so relevant evidence is easier to understand and use.

Ryan brings 15+ years of registered nursing practice and senior clinical and NDIS leadership experience. This connects clinical detail with the practical support needs recorded by your treating professionals.

What you can receive
• Plain-language explanations that preserve the original findings.
• Agreed evidence summaries, timelines and document indexes.
• Missing-information checklists and focused questions for your clinicians.

Consent, authority, secure transfer, outputs and paid scope are agreed before review. NDIS funding depends on the activity and claiming conditions; it is not automatic. Private fees and NDIS arrangements are separate. Initial-application work requires an agreed payment pathway before it starts; later NDIS approval does not authorise payment for earlier services. See Fees & funding.

Original clinical meaning is preserved. This service does not replace clinical assessments, create new diagnoses or clinical findings, prescribe, or provide legal representation. Further evidence may be needed from treating professionals. The NDIA decides access and funding; outcomes are not guaranteed. National virtual access. Digital language assistance is a separate option, subject to suitability, availability, consent, scope and cost. Certified translation requires appropriately qualified professionals.

Send a brief enquiry or call 0459 173 805. Leave medical records, NDIS numbers and sensitive participant details out of the initial enquiry. We confirm suitability and capacity first. Your non-excludable consumer rights remain.

Translation between languages is separate from explaining medical terminology in plain English. See our digital language assistance options.
