How referrals work

Start a referral

For participants, families and authorised professional referrers across Australia.

1. Start with your own email address and phone number and the broad type of support needed. Other initial details are optional. On the enquiry form, choose Participant referral.

Please leave participant identifiers, NDIS numbers, medical records and other sensitive information out of the initial enquiry.

2. We discuss suitability, current availability and your intended funding pathway. As an unregistered NDIS provider, VCC works with eligible self-managed and plan-managed participants. We do not provide plan management or NDIA-managed supports.

3. Before records are shared, we confirm participant consent, your authority, the agreed purpose and an appropriate transfer method. Before services begin, scope, deliverables, fees and funding arrangements are agreed.

Enquiries are reviewed during business hours, Monday to Friday, 9:00 am–5:30 pm Queensland time. Capacity and a response time for the proposed service are confirmed case by case. If you have not received a reply within two business days, please phone 0459 173 805.

An optional free suitability call of up to 30 minutes is subject to availability and carries no obligation. Substantive record review, analysis and reports require an agreed paid scope.

Private health consultancy is a separate option for individuals, families, providers and organisations across health care. An NDIS plan is not required. For an organisational review, choose Provider consultancy; contact VCC for individual or family enquiries. A written proposal confirms scope, deliverables, fees and payment terms before work begins. See Fees & funding for enquiry and NDIS funding information.

For a referral capability statement, contact VCC using a brief enquiry. For immediate danger or urgent medical assistance, call 000. VCC is not an emergency response service.
